چکیده

Background Myocardial perfusion and strain (in particular longitudinal strainLS) are reduced in severe aortic stenosis (AS). It is thought that reduced perfusion occurs in the subendocardium secondary to increased LV pressure, leading to fibrosis and reduced LS. This relationship between perfusion, diffuse fibrosis and multidirectional myocardial strain has not been previously investigated in moderate (or severe) AS. We hypothesized that impaired myocardial perfusion occurs in patients with moderate AS, leading to a degree of diffuse fibrosis (reflected in increased non-contrast T1 values) and impaired longitudinal strain.

Severe aortic stenosis has blunted myocardial T1 relaxation response to vasodilator stress: a cardiac magnetic resonance adenosine stress test study

Background Aortic stenosis is characterized by impaired myocardial perfusion reserve due to coronary microvascular dysfunction. T1 mapping is sensitive to myocardial water content of both intraand extracellular in origin, but the effect of intravascular compartment changes on T1 has never been assessed previously. We aimed to assess the effect of adenosine-induced vasodilatation on native (pre-...

Circumferential strain predicts major adverse cardiac events independent of myocardial perfusion in asymptomatic aortic stenosis

Background Asymptomatic aortic stenosis (AS) remains a challenging clinical entity characterised by adverse left ventricular remodelling, microvascular dysfunction and sudden cardiac death. Previous studies have shown reduced myocardial strain in patients with asymptomatic aortic stenosis, however the relationship between impaired myocardial strain, ischaemia and major adverse cardiac events (M...

Human non-contrast T1 values and correlation with histology in diffuse fibrosis

BACKGROUND Aortic stenosis (AS) leads to diffuse fibrosis in the myocardium, which is linked to adverse outcome. Myocardial T1 values change with tissue composition. OBJECTIVE To test the hypothesis that our recently developed non-contrast cardiac magnetic resonance (CMR) T1 mapping sequence could identify myocardial fibrosis without contrast agent. DESIGN, SETTING AND PATIENTS A prospectiv...

Adenosine stress native T1 mapping in severe aortic stenosis: evidence for a role of the intravascular compartment on myocardial T1 values

BACKGROUND Myocardial T1 relaxation times have been reported to be markedly abnormal in diverse myocardial pathologies, ascribed to interstitial changes, evaluated by T1 mapping and calculation of extracellular volume (ECV). T1 mapping is sensitive to myocardial water content of both intra- and extracellular in origin, but the effect of intravascular compartment changes on T1 has been largely n...
